* Combo processing improvements.
Now it is possible to use ModTap and LayerTap keys as part of combos.
Overlapping combos also don't trigger all the combos, just exactly the
one that you press.
New settings:

* Remove the size flags from combo_t struct boolean members.
This in the end actually saves space as the members are accessed so many
times. The amount of operations needed to access the bits uses more
memory than setting the size saves.

Stuff combo's `disabled` and `active` flags into `state`. Possibly can
save some space.
* Add more examples and clarify things with dict management system.
- Simple examples now has a combo that has modifiers included.
- The slightly more advanced examples now are actually more advanced
instead of just `tap_code16(<modded-keycode>)`.
- Added a note that `COMBO_ACTION`s are not needed anymore as you can
just use custom keycodes.
- Added a note that the `g/keymap_combo.h` macros use the
`process_combo_event` function and that it is not usable in one's
keymap afterwards.

* Fix sneaky infinite loop with `combo_disable()`
No need to call `dump_key_buffer` when disabling combos because the
buffer is either being dumped if a combo-key was pressed, or the buffer is empty
if a non-combo-key is pressed.

This converts the array that the Swap Hands feature uses to use PROGMEM,
and to read from that array, as such. Since this array never changes at
runtime, there is no reason to keep it in memory. Especially for AVR
boards, as memory is a precious resource.

Currently, boards and keymaps that define NO_ACTION_MACRO/FUNCTION unconditionally
will not compile with LTO_ENABLE (#8604). This fixes the issue by moving the
definitions from common.mk to action.h, which enables us to check for previous
definitions of those macros (this cannot be done in a makefile).

Since now NO_ACTION_MACRO/FUNCTION are defined as needed in action.h (which is
included by quantum.h), checking for LTO in keyboard and user code is no
longer required.

I think a few people have reporting issues with it working properly, and it may be a timing issue. The 'register_code' uses this sort of delay in some of the functions, and
this is probably why.
Adding the 100ms delay should hopefully fix any issues with it.

This is an inelegant hack for #2522 but makes things work. Basically we give `action.c` a chance to handle the hold event early so that we can swap the keyboard for later keys. Later, to allow the hold to happen again quickly we nuke the key record so that tapping is reset. I tried to find a cleaner way, honestly.
This adds an action, `ACTION_SWAP_HANDS`, that swaps the the keys on the keyboard across a keymap-defined hemisphere in order to support one-hand typing without requiring a separate one-handed layer. See updated `doc/keymap.md` for more information.

If one wants to temporarily disable the key cache (for example because
it interferes with a macro), `disable_action_cache` must be set to
`true`. `process_action_nocache` is a simple wrapper doing just that for
a single call.
